The new form is up to the Club Mark standard and is to be used for both junior and adult members on any trip. We have been advised by the BCU to not have separate forms i.e. youth, Private, Members and Club trip versions hence this new generic version has been produced.

The form is in MS Word format so that trip organisers can enter trip details digitally then print/email the form for participants to complete and return the relevant section to the trip organiser.
